Are you passionate about making a difference in the lives of individuals with disabilities? DSSW is seeking a dedicated and compassionate Case Manager to join our Beaumont team! What You’ll Do:

  • Oversee day-to-day operations of the managed waiver program.
  • Monitor and support services for your assigned caseload.
  • Enroll new applicants and ensure compliance with DBMD program standards.
  • Build strong, positive relationships with clients, families, vendors, and team members.
  • Provide crisis intervention and on-call support as needed.
  • Travel to client homes and community activities in the Beaumont and Houston areas.

What You Bring:

  • Education & Experience (one of the following):
    • Bachelor’s in health/human services + 2 years’ experience
    • Associate’s + 4 years’ experience
    • High school diploma + 6 years’ experience
  • Strong communication skills and fluency in client communication methods.
  • Proficiency in MS Word, Excel, and database systems.
  • Reliable transportation, valid TX driver’s license, and auto insurance.
  • CPR/First Aid certification (in-person only).
