Position Summary
The PAS Care Team Assistant II is responsible for coordinating home care referrals and scheduling field staff visits in accordance with the established plan of care. This position serves as a central point of communication between physicians, case managers, staff, and clients, ensuring seamless transitions into home care and timely delivery of services. The role requires strong organizational skills, effective communication, and the ability to manage multiple priorities in a fast-paced healthcare environment.
Essential Duties & Responsibilities
- Receive and process home care referrals and physician orders via telephone and facsimile, serving as the primary communication liaison among physicians, case managers, staff, and clients.
- Provide information to prospective clients and referral sources regarding Lifespan Home Care services and general home care options.
- Collect, verify, and communicate clinical, demographic, and payer information to ensure accurate admissions and smooth initiation of care.
- Maintain and manage the master schedule of all field staff visits, assigning and adjusting visits based on acuity, skill requirements, patient needs, and staff availability.
- Schedule or reschedule visits as needed when staff are unavailable or client needs change.
- Prepare referral, census, and statistical reports for the Supervisor and/or Administrator as assigned.
- Collaborate with Corporate Human Resources to ensure employee files are complete, up to date, and reviewed annually as required.
- Assist with new hire orientation and documentation completion, as needed.
- Ensure assigned programs comply with all federal, state, and agency regulatory requirements.
- Coordinate with the Payroll Department to ensure accurate employee time records.
- Conduct client admissions and evaluations as assigned.
- Provide initial orientation to clients and newly assigned attendant staff.
- Submit required reports within established timeframes.
- Communicate promptly with clients’ program caseworkers (including via Form 2067) regarding service interruptions or client-related issues.
- Track unstaffed hours weekly and document recruitment and staffing efforts.
- Participate in staff meetings, case conferences, and in-service training activities as directed.
- Travel as needed for client visits or hourly care as required.
- Maintain strict confidentiality in accordance with HIPAA and agency policies.
- Perform other duties as assigned by the Supervisor or Administrator.

Education & Experience
- High school diploma or GED required.
- Six (6) months to one (1) year of staffing or scheduling experience in a home health agency or medical setting preferred.
- Prior home health experience strongly preferred.
- Ability to follow written and verbal instructions accurately.
- Proficiency in Microsoft Word and Excel; ability to learn agency-specific software.
- Typing proficiency of 35 words per minute with 80% accuracy, including Excel accuracy.
- Reliable transportation, a valid driver’s license, and current auto liability insurance required.

- Ability to lift objects weighing more than 25 pounds.
- Finger dexterity for keyboard and office equipment use.
