Position Summary
The PAS Case Manager is responsible for managing ancillary programs—including STAR+PLUS, STAR Kids, PHC, CAS, FC, MDCP, PCS, VA, AACOG, and Private Duty/Personal Assistance/Sitter Services—and ensuring the delivery of high-quality personal care services to patients and families. This role operates in accordance with agency policies, regulatory requirements, and scope of practice while supporting operational, clinical, and administrative functions.
Key Responsibilities
- Supervise all Personal Assistance Services (PAS) and Community Care Program employees in the planning and coordination of patient care in collaboration with physicians’ plans of care and applicable state program or Managed Care Organization (MCO) Individual Service Plans (ISP).
- Assist with billing for all ancillary program patients in compliance with agency billing policies.
- Coordinate and collaborate with billing, scheduling, and office staff to support smooth daily operations.
- Partner with the Human Resources Manager to ensure employee files are complete, accurate, and reviewed annually.
- Ensure all assigned programs comply with regulatory guidelines and agency policies.
- Work closely with the payroll department to verify accuracy and timely submission of employee time records.
- Complete required supervisory visits within regulatory timeframes and document supervisory notes promptly in the clinical record.
- Communicate proactively with state program representatives, MCO caseworkers, and service coordinators regarding service interruptions and client issues.
- Participate in recruitment, training, and ongoing in-service education to ensure high-quality staff coverage.
- Track unstaffed hours weekly and document recruitment and hiring efforts.
- Assist the Administrator with budget planning and program implementation.
- Participate in case conferences, in-services, meetings, committees, and agency programs as assigned.
- Support Human Resources with the management of personnel and health folders for home health team attendants.
- Maintain confidentiality in all matters related to patients, staff, and agency operations.
- Perform additional duties as assigned by the Administrator or designated leadership.

- Reliable transportation, valid driver’s license, and agency-required auto liability insurance are required.
Education & Experience
- PAS program management experience preferred.
- Minimum of two (2) years of supervisory experience or completion of two years of college coursework.
- Experience working with HHS caseworkers and MCO service coordinators preferred.
- Strong written and verbal communication skills.
- Basic computer proficiency required, including MS Word and Excel; ability to learn agency-specific software.
- Typing proficiency of 35 words per minute with 80% accuracy.
- Ability to read, write, and communicate effectively in English; bilingual English/Spanish preferred.
Physical Requirements
- Ability to work a minimum of 40 hours per week.
- Ability to travel for client visits, hourly care, and community activities as required.
